An SMW Personal Lifestyle Manager provides dedicated personal support to co-ordinate daily life, routines, and lifestyle preferences. Through holistic oversight, organisation and planning of peronal and household matters, this program delivers continuity and reliability through a single trusted partner to seniors and their families.

Our programs do not include any medical, clinical or personal care services. They are also designed to have some level of built-in flexibility, with the final structure tailored and confirmed during the consultation process.

We come to you for a confidential discussion to understand your needs, how your life currently looks, what matters to you and what support would be most helpful.

 

This is a no-obligation in-home discussion focused on observing, listening and understading.

Recommendation

Based on the consultation, a tailored monthly program is proposed outlining the type of support, visit rhythm, and how any flexibility you need can be built in.

 

Everything is explained clearly, including pricing, so expectations are set from the start.

​

Depending on the program, we may also introduce you to a selected Lifestyle Assistant.

Continuation

Support begins on an agreed start date and duration with a consistent routine established, adapting as needs change. 

​

Regular reporting starts to agreed rhythm, including quality assurance and regular program reviews

 

The focus is on continuity, familiarity,  consistency and ensuring life continues comfortably at home.

Our purpose.

We're on a quest to help older Australians continue living life in their own home, as an alternative to moving into residential aged care when they don’t need to.

​

The purpose behind this is to help protect dignity, independence and wellbeing at a time in life when familiarity and the certainty of daily routines and rhythms create a sense of comfort that people both need and deserve.

​

We are not about disrupting aged care or trying to reinvent how services are delivered.

​

SMW is about refocusing in-home support on the outcomes that actually matter to the person receiving it, not simply the activity of providing it.

​

And we're about offering real choice to those who are locked out of government-funded care, and who simply want to continue the lifestyle they have already built.
