A life that's organised and active.
We're here for seniors starting to find parts of everyday life harder to stay on top of, and for families stepping in to keep things running.
Sage, Morgan & Wren provides a private, independently-funded alternative to relying on family or juggling multiple service providers.
Our structured, flexible programs cover both home life, and ongoing connection to social and community life. The privately-funded nature of our service gives clients the control and flexibility to tailor programs, something that isn’t always possible within government-funded services.
A carefully selected member of our team does the work, providing one point of contact and accountability, making sure that everything is handled consistently, and to the highest standard.
Seniors feel the change, but the available options for support don't fit.
Keeping the home running smoothly is a strain.
Household routines become harder and take more effort, things slip, and the home feels less organised and less comfortable.
Energy and motivation for outings and visitors is low, some of the established social habits and connections begin to fall away.
Socialising or meeting people takes too much effort.
Accessing the aged care system doesn’t feel like the right fit, and most people are not ready for that step.
Aged care feels like too much, too soon.
Family feels the strain, but the available options just add to it.
Support from family starts to become ongoing.
What started as occasional help for an elderly parent has become ongoing, adding a layer of stress to already stretched family members.
Constant anxiety about whether enough is being done.
With no structure or plan, it's harder for busy adult children to stay on top of everything. Inevitably, things get missed, guilt builds that not enough is being done..
Juggling multiple service providers adds to the stress.
Different people, from multiple providers, come and go, with no consistency in the work or who owns the full picture, turning adult children into reluctant supervisors.
Core pillars upholding every program we deliver.
Structure & planning.
Without structure, things get missed, pile up, or even forgotten. Structure keeps work organised, on track to agreed goals, and helps anticipate, and avoid, problems.

One person doing the work.
Rather than multiple people going through the home, we assign the work to one SMW team member, who becomes familiar face for seniors and the family.

Consistency & familiarity.
Ad-hoc support by multiple people can be inconsistent and unpredictable. Structured programs, delivered by a familiar face helps restore consistency, and piece of mind.

Each program is built to keep the parts of home life that matter most working properly, with consistency, accountability, and control in your hands. All programs run month-to-month, with flexibility to adapt as circumstances change, ensuring nothing is missed or left to chance.

Please note that our programs do not include medical, clinical or personal care services. If you require these services, please let us know as we may be able to work alongside your chosen provider.
Enhance any program with our lifestyle add-ons.

Pet Lovers
Secured, dedicated time for the regular support of your pets, from walking and feeding to transport and vet visits.

Community Connections
Extra time for planning, co-ordination and companionship to regular social and community events beyond the home.

Weekend Wonders
Help with special occasions and busy weekends, providing direct support for hosting family, at-home events, or simply ensuring plans run smoothly.

People you'd trust in your home.
The SMW team members we bring into a client's home are carefully chosen, not just for what they can do, but for who they are.
They are not traditional carers. We recruit from outside the sector, often looking for people with deep service-based backgrounds, and invest in them to make sure clients have someone familiar, consistent, capable and trusted.
They're smart, calm,pro-active and take pride in doing things properly. They notice what others miss, respect how clients choose to live their lives, and understand the importance of discretion, consistency and trust.

It all starts by understanding what matters to you.
Consultation
We come to you for a confidential discussion to understand your needs, how your life currently looks, what matters to you and what support would be most helpful.
This is a no-obligation in-home discussion focused on observing, listening and understanding.

Reccomendation
Based on the consultation, a tailored monthly program is proposed outlining the type of support, visit rhythm, and how any flexibility you need can be built in.
Everything is explained clearly, including pricing, so expectations are set from the start.

Depending on the program, we may also introduce you to the selected SMW team member who will be looking after you.

Commencement
We begin on an agreed date, establishing a regular routine in the first month, with a view to adapting as needs change or are reviewed.

Regular reporting also starts to an agreed cadence, including quality assurance and regular program reviews
The focus is on creating a sense of continuity, familiarity, consistency and ensuring life continues comfortably at home.

Our Purpose.

We're on a quest to help older Australians continue living life in their own home, as an alternative to formally entering the aged care system, or even moving into residential aged care when they don’t need to.

The purpose behind this is to help protect dignity, independence and wellbeing at a time in life when familiarity and certainty in the daily routines and rhythms of life create a sense of comfort that people need and deserve.

We are not about disrupting aged care or trying to reinvent how services are delivered.

SMW about refocusing traditional home-based support, not on the completion of task lists, but on tangible outcomes that actually matter to the person receiving it.

And we're a firm that offers real choice to those who are locked into, or locked out of, a rigid government-funded care system, and who simply want to continue the lifestyle they have already built.
